Abstract

OBJECTIVES: to analyse the evolution of Health Related Quality of Life (HRQL) in coronary patients (CP) and to identify predictive factors influencing this evolution in a situation with missing data and time-dependent variables.
DESIGN: prospective study with repeated measures.
SETTING AND PARTICIPANTS: a total of 175 CP were included. General Estimating Equations (GEE) models were used to assess the evolution of HRQL in these patients. These models, not commonly used in this context, are applied here as an alternative to traditional techniques that do not handle missing data and time-dependent covariates properly.
MAIN OUTCOME MEASURES: HRQL assessed by SF-36v1 Questionnaire at baseline, 3 and 6 months after discharge.
RESULTS: role physical, bodily pain, general health, vitality, and the physical component summary of SF-36 improved over the follow-up. Being woman, older, and having higher scores on GHQ-28 were associated to a decrease in HRQL throughout time. Previous history of coronary heart disease, comorbidities, revascularisation, rehospitalisation, and episode of angina had a negative impact on HRQL, especially between 3 and 6 months after discharge.
CONCLUSION: the analysis of the evolution of HRQL with a longitudinal approach using GEE models shows the predictive effect of the variables analysed during the follow-up, including the time itself and time-dependent covariates such as the evolution of mental health. In addition, it allows to particularise the predictive effect of covariates at each period within the follow-up.

Riassunto

OBIETTIVI: analizzare l’evoluzione della qualità di vita correlata alla salute (HRQL) in pazienti coronarici (PC) e individuare fattori predittivi che influenzano questa evoluzione in presenza di dati mancanti e variabili dipendenti dal tempo.
DISEGNO: studio prospettico con misure ripetute.
SETTING E PARTECIPANTI: sono stati inclusi in totale 175 pazienti coronarici. Modelli di equazioni di stima generalizzate (GEE) sono stati usati per valutare l’evoluzione dell’HRQL in questi pazienti. Tali modelli, non utilizzati frequentemente in questo contesto, sono impiegati in questo caso come alternativa alle tecniche tradizionali che non trattano propriamente dati mancanti e variabili dipendenti dal tempo.
PRINCIPALI MISURE DI OUTCOME: HRQL valutato tramite questionario SF-36v1 all’arruolamento, dopo 3 mesi e 6 mesi dalla dimissione.
RISULTATI: durante il follow-up sono migliorati ruolo fisico, dolore fisico, salute generale, vitalità e il riassunto della componente fisica dell’SF-36. Le condizioni associate a una riduzione dell’HRLQ nel tempo sono essere donna, essere più anziano e avere punteggi di GHQ-28 più alti. Eventuali precedenti di malattie coronariche, comorbidità, rivascolarizzazione, riospedalizzazione ed episodi di angina hanno avuto un impatto negativo sull’HRQL, specialmente tra i 3 e 6 mesi dopo le dimissioni.
CONCLUSIONE: l’analisi dell’evoluzione dell’HRQL con un approccio longitudinale usando modelli GEE mostra l’effetto predittivo delle variabili analizzate durante il follow-up, incluso il tempo stesso e le covariate dipendenti dal tempo, come l’evoluzione della salute mentale. Inoltre, tale strategia rende possibile dettagliare l’effetto predittivo delle covariate in ogni periodo all’interno del follow-up.
